Student File Cover Page. This form contains all of the basic information we need on each student. 

(NOTE: for outreach programs, please list your program as the emergency contact / authorized drop off and pickup for this form)

Liability waiver.  This is the liability waiver all students must sign before participating in a Simply Circus program.

Photographic / Video release form.  This photographic and video release form must be signed before we can photograph or videotape a student.  Please note that in addition to filming of student shows, we make extensive use of photography and videography in our classes and camp programs.
